Admissions & Fees
To make an application to Midtown Christian Academy there is a $150.00 application fee per child. There is a processing fee for returning students of $150.00 per child in August of each year to hold a space for your child the following year.

Before admission to Midtown Christian Academy, the following must be on file in the administrative office.
Registration form with registration fee (non-refundable)
First Month’s tuition
*Tuition agreement
*Release Form
*Emergency Card
Form 121 (immunization record)
ENROLLMENT INFORMATION
Children’s Record
According to the Mississippi Department of Health, the following documents must be on file in our office before your child enters the school. Your child may not start school without the following on file:
* Application/Enrollment Form
* Registration fee Paid (Non-Refundable)
* Immunization Form
* USDA Form
* Child Information Form (includes emergency contact information, health and allergy information, transportation information, field trip permission, sick child compliance statement, and acknowledgement of receipt of Parent Handbook)
It is the parents’ responsibility to keep all forms updated with all necessary telephone numbers and any new information pertinent to their child’s well-being.
